Full policy

This policy explains how Phoenix Behavioral Pathways LLC (“we”, “us”) collects and uses information through behavioralpaths.com. If you have questions, email info@behavioralpaths.com or call 602-200-6758.

How our contact form works

Our contact form is provided by SimplePractice, the secure practice-management system we use for scheduling, client records and appointment reminders. What you enter goes directly to SimplePractice, not to this website, and we have a business associate agreement with SimplePractice. If you email us instead, we use your message to reply to you and schedule a first conversation. Please don’t include detailed health information in your first message. We’ll talk through your needs privately. If you become a client, you will receive separate documents at intake that explain how we protect your health information.

Information we collect

  • Contact form: the form opens in a window from SimplePractice and asks for your name, email, phone number, date of birth, the state you live in and your message. It goes directly to SimplePractice, not to this website. SimplePractice’s own privacy policy also applies.
  • Website analytics: we use Google Analytics through Google Tag Manager to see how visitors use the site, for example which pages are viewed. Google Analytics uses cookies and receives your IP address and device information. We don’t send the contents of the contact form to Google Analytics.
  • Fonts: our pages load fonts from Google Fonts, which receives your IP address.
  • Text messages: if you agree to receive texts, our SMS Disclosure explains how we use your mobile number.

How we use information

  • To reply to your request and schedule appointments
  • To send the text messages you agreed to receive
  • To understand how the website is used and improve it
  • To keep the website secure and meet legal requirements

How we share information

We don’t sell or rent personal information, and we don’t share it for marketing. We share it only with service providers that help us run the website and our communications. Our communications are powered by SimplePractice (for client portal notifications and appointment reminders) and Zoom Video Communications, Inc. (for office phone and backup SMS routing). We also share information with our website host, our email provider and Google (for analytics and fonts), or when the law requires it.

How long we keep information

Messages sent through our contact form are kept in SimplePractice. If you become a client, your records are kept for as long as Arizona law requires for health records. Emails sent to us are kept only as long as we need them to respond to you, unless the law requires us to keep them longer.

Our website, email and voicemail are not monitored around the clock. If you are in crisis, call or text 988, or call 911 if you are in immediate danger. See Crisis Resources.
